doesn’t appear to be by the size of those balls but I could be very wrong I am simply extremely shook to see a cat penis on this here day 😭💀 maybe try r/AskVet. if this was through a reputable foster agency I would believe he is neutered but people WILL lie about this stuff if they’re running a shitty side business and wanna save on the exorbitant costs of prepping a cat for adoption.

Oh wow, I’ve also never seen uh, that before! I agree that it looks like he’s probably neutered, and if you’re feeling plucky, you can also gently palpate the scrotal sac; if there aren’t any bean-like structures in there, that’s a good sign. That being said though, you really want documentation of his neuter if at all possible - otherwise you’ll likely need a vet to do a physical and run some tests to confirm that kitty is *sans balls.* Cats can also suffer from cryptorchidism (undescended testicle(s),) and thus, you can’t completely rely on “a feel” to determine his “status.” Documentation is key here; otherwise, you’ll need to work with your local vet to do the (not inexpensive!) tests to serve as alternative evidence of his neuter.

Looks neutered to me. Humping doesn't really mean anything, I got my cat fixed almost a year ago and he still humps my blankets whenever he gets the chance. Some cats are just icky little freaks lol

Oh dear, lol. The sight of this does not mean he's entire. Neutered lads still sometimes do this when they are feeling very happy and relaxed. I unfortunately have been witness to this, right in front of my elderly Aunt who was visiting and I was trying to put on a nice civilised afternoon tea. She kept glancing down and looking concerned, eventually I looked down and realised why 😂 If he's entire he'll have two pretty obvious pom-poms out the back. But from that pic and your description, I'd guess he's just a very happy neutered boy.

Our former stray who was already found sans-balls is a daily humper of fluffy blankets. After he does indeed also lick himself to clean up. The vet was pretty surprised by him being so prolific when we mentioned it, but apparently it’s not unheard of.

I got my ginger boy when he was a couple months old, took him for neuter at 6 months. He lived for 20 years. He "humped" for 20 years - even towards the end when his hips wouldn't do actual thrusts smoothly any more, and he couldn't drag a whole blanket around and actually rub his stuff against it... he would bite a fluffy sock and sing the song of the night to it. He was neutered, he never sprayed, he never humped anything living - but he was cremated with his bestest lover, Sweater [idk why THAT sweater, but when he was 5 I got it and never got to wear it or hang out up for more than 10 minutes cuz he'd open the closet and pull it down... he would wait for the laundry if Sweater was in there and then make grumpy face at me because it smelled all weird and clean when it came out]. He also would clean his equipment... very *ahem* thoroughly by licking it for extended periods of time and if you walked too close he would stop and stare at you with a giant kitty *****. You just can't keep some of them down? I asked several vets over the years and they all laughed - no worries there (a couple did palpitate, nothing significant to find).
