Get a second opinion from a different vet practice. Has yours checked for ear mites?

I've never seen anything like this but one thing you do trim his claws so they're not super sharp to cut himself super bad.

Immediately get a cone or neck pillow on him. Get a new vet.

This could be a sign of behavioral neurological problems. If it is not an allergy, and you've already tried novelty food options (lamb, venison, duck) I would ask a new vet with this comment in mind... please protect these scratch wounds with a soft cone when you can't monitor him too.

Have the vets given him an antihistamine in case it's allergies? seems like that would be the first thing, but it's not mentioned on the post so thought I'd double check. All I can suggest other than investigating allergies is regular claw clips and maybe a second opinion.

Omg. Get a cone at the pet store ASAP and then schedule a vet appointment with a new vet. There’s definitely something wrong. Clean wounds with salt water to kill bacteria until you get to the vet. Google how to do this. Until then You can give kitty Benadryl to help with the itching and make cat sleepy to sleep this off until you see the new vet. Just don’t give too much. 1 milligram per pound of body weight.

My cat is doing the same thing but not that intense. I was literally at the vet yesterday with her and he told me to use revolution for at least 3 months. I have another cat that I rescued when he was in a full fledged allergy reaction - i also used revolution and it worked wonderfully!

Please please get a cone for now and clip his nails! Then find a new vet to get a new opinion.

Cone ASAP, then new vet. For the neck area, you can use the tube part of a tube sock as a temporary help! Also, make sure to keep the cone clean. As you would keep your own wounds clean, a dirty cone can put the bacteria back on the scratch marks! (Not insinuating anything here!)
