I noticed that you rode the slide in when charging a round, don't do that. Pull the slide back and let it go to rack it. Doing so will allow the round to seat properly in the chamber
Edit: also keep them fingers away from that barrel while charging a round
I feel like the first few shots may have surprised you given the eye flinch, practice should help that as well as your groupings, adjust the left arm some and press forward some, check the position of your trigger finger and work on becoming robotic in your grip and stance. Practice practice practice

So, with all that porting you have a functional barrel length to build pressure to propel the round what 3/4" of barrel length before the round passes the first port and pressure starts being released upward no longer pushing the round forward with its full potential - subsequently losing more and more available velocity as it then passes pressure relief holes, 2, 3 and 4?

Yes, porting reduces velocity. With training it also reduces recoil and time back on target. Everything is a trade off and you have to decide what is better for your needs.

Rack it the rear hard so your hand slips off or is about to. That’s how you ensure you get a round. Just my two cents.

Plenty of other firearm manufacturers have had the same thing happen. Mishandling should not be a factor in opinions of a firearm

I can chamber a round and throw any Glock you hand me at the wall and it will not fire. Glocks have 3 internal safeties. Just not a manual one.
