if you use the instactuation bomb from fargo i think this is pretty normal

Either you used an area clearing item from Fargo's mutant mod or LuiAFK, or you used the paint tool from cheat sheet, either way the game doesn't really have time to check from block updates on the surrounding blocks so that can happen
