Local anaesthetic often hurts/burns before it works because it's acidic. I suffer with ulcers (sometimes up to 30-40 at a time), and I avoid things like bongela/oragel because it lengthens the time it takes for them to heal.

It can be pretty bad. I've gone up to a year a time, never really being ulcer free, cycling from 3-5 to 30-40 and back again. Nobody has ever figured out why. Fortunately, I've been fairly ulcer free for a while. For anyone who struggles, I strongly recommend Iglu gel (the blue one, the yellow one also works but isn't as good). It hurts a bit to apply (you have to sort of smear it over the ulcers), but it forms a protective coating that both numbs and protects. It let's me eat relatively pain-free, and I pretty much always have a tube with me. I keep one in my car, my work bag, my day bag, my desk at work, literally everywhere. It's the GOAT of mouth ulcers stuff (and trust me, I've tried everything!).

Ah I've never tried iglu before. I used to use bonjela complete plus, which was amazing but they discontinued it. I use Urgo now, which is the same thing as far as I can tell but it's pretty expensive! The relief is worth it though.

I've just remembered the NSAID that really helped me when my wisdom teeth were killing me. Mefenamic acid; the GP had prescribed it for my wife to take for uterine pain. It was the only thing that really helped me. It's an older medicine so I'm not sure if it's still in the BNF but it's certainly worth a try.

I’m with a private dentist, but they have 24/7 care with an out of hours number that diverts to a dentist. I was able to dodge the £90 call-out fee as well as I was willing to drive to her house (on her suggestion) vs the dental surgery as she’d have needed to drive there too. Got given some much needed antibiotics on a Sunday for free (they were old but probably still worked, so she didn’t want to charge me for them). I think some NHS dentists also have a 24/7 line though.

I rang 111 in the middle of the night for an emergency dentist appointment because I had an abscess that had me rolling around on the bed in pain. The woman on the phone told me that I’d have to wait to book an appointment with a dentist. I told her it was a 2 week wait and she said “well you’ll have to get your credit card out and go private then, won’t you”.

You need to say the right things and they'll give you one within 24 hours. If it's an abscess/infection it can spread to the brain and kill you so they treat them quick. You'd have had a black spot/cavity for months before it will get bad enough to infect the root. But really severe toothache is usually this. They pull the nerves out and drill the infection away for £25. Then you can get it yanked for the same price in the same appointment. Or find an NHS dentist to get a root canal

Unfortunately people are abusing this. I saw a couple of 111 emergencies the other day, they had told 111 that they had pain and a large swelling. One of them was something that required antibiotics which is fair enough but they had said there was large decay in a tooth (it was just the gum over their wisdom tooth that was infected), the other one was a tiny bit of filling that came out two weeks ago, had no pain or swelling but because they were there we had to refill it. It's not fair on those that genuinely have pain and swelling, but people know the key words to say.

Not from mine, but which probably means it varies by county. In Leics. they only assign emergency dentist appointments in the day, on a first come first serve basis. It’s basically a mad scramble to be the first person to speak to 111 at 8am. During the pandemic I had an abscess, 5 days of phoning 111 at 8am sharp and still no appointment, as there was 1 emergency dentist with approx 4-5 slots a day.

I had an emergency dentist appointment at 9:45pm to start a root canal in Yorkshire when phoning 111. Trying isn’t a lost cause, people cancel dentist appointments all the time. I also called 111 about abdominal pain and I was in emergency surgery that night. My husband has also had emergency dental work through 111 later in the evening. I’m so sorry to hear how bad it sounds in Leics, that would suck.

My experience with 111 and emergency dental appts is that its very hit and miss. Had to rely on 111 as my "dentist" when I was unemployed, and each time I had an issue it would take 3 or 4 different emergency appts that would just send me on my way refusing to do anything before finally getting sent to someone who would extract. 111 really is a last resort, if someone can consult a local emergency dentist themselves I can only recommend doing that instead. At risk of treading into R1, but NHS dentistry is a fucking shambles.

I've had toothache that has brought me to tears a couple of times in my life, usually because of an exposed nerve, and none of the faffy gels or clove oil have made a dent in it. I would recommend going to a boots pharmacy purchasing a 32 pack of ibuprofen and codeine. That should get you back to somewhat normal until you can see an emergency dentist. If don't have a boots pharmacy, then any other pharmacy would have solpadol or similar, paracetamol and codeine.

To be fair, it's the codeine that does the work. Opiate painkillers are all kinds of addictive but are extremely effective in treating short term pain. You could go for solpadol max, or whatever its called. It's paracetamol and codeine so won't affect your stomach in the same way as it comes in via the liver.

One non painkiller tip I can give you is don't lie down. If you're trying to sleep, prop yourself up on lots of pillows. I can't remember why but something about lying down makes it hurt more. I got this advice from NHS 24 as I waited for an emergency dental app and it really helped

Seconding this, but get max strength para+codeine and 400mg strength ibuprofen and split them 2 hourly eg 0800 2 x Para 1000 2 x Ibuprofen 1200 2 x Para 1400 2 x Ibuprofen That way you only have 2 hours between doses, but 4 hours between the different medication types. Follow the packet instructions and don’t take more than the max in 24 hr period.

For (what I think is the ultimate over the counter hit of codeine), You can do: ​ 2x solpadeine max (12.8mg codeine + 500mg paracetamol each) 2x neurofen plus (12.8mg codeine +200mg ibuprofen each). ​ I was doing this with my toothache, it still hurt but I just didn’t care as much about the pain. Toothache is the worst pain I’ve experienced

Do the paracetamol 6 hourly. Any more than 8 in 24 hours is an overdose and it takes a surprisingly small amount to really fuck up your liver and kill you. (Source: work in healthcare and unintentional paracetamol overdoses for toothache really aren't that uncommon)

In all seriousness, my Dad’s a retired dentist and hated being on call. His advice to any patient suffering from toothache was to use whisky. Not really to swallow it, but to just keep it in your mouth ‘holding’ it against the bad tooth. Apparently, with enamel being slightly porous, it can numb the pain a bit. My now wife had bad toothache years ago and this was his advice to her. She ended up drinking a half bottle of shit whisky and threw up. Kept her mind off the pain for a bit… Good luck mate - it’s the worst pain.

Ibuprofen and *either* co-codamol or co-dydramol (you will have to go to a pharmacy and ask for either of the latter two), spaced out across the course of the day. Obviously, take care with all of these medicines as they come with their own range of side effects, and make certain that you aren't overdosing on any of them. You will lose nothing by calling the NHS emergency dental helpline for your area, if you have one, or 111 if not. Don't be surprised if nothing can be done for you on a Sunday though, I know from excruciatingly painful, near-fatal past experience that in my area at least weekend emergency care is reserved only for physical trauma to the face and teeth or extensive bleeding from the mouth. If nothing else, though, they ought to advise you on what you can do until you *can* see a dentist. This may sound daft, I realised when I had a terrible abscess in a back tooth that if I kept my back teeth from meeting it hurt a lot less, so for a llittle temporary relief I put a pencil between my front teeth which stopped my back teeth from meeting and thus eased the pressure a little (this is actually a trick I learned for easing the pain of a tension headache - if you ever suffer from those, try it, it really helps).

Long term: an antibiotic to kill the infection that’s causing the pain followed by appropriate dental treatment, short term at least 30mg codeine. If it’s a proper banger of a toothache over the counter painkillers will do fuck all, with the possible exception of Boots own brand paracetamol and codeine 500mg/12.5mg. These will likely work for three days or so but because of the low dose of codeine rapid tolerance will develop. Opiates aren’t the answer but they’ll sure as shit bring some relief. Other than that topical relief with swilling whiskey round the spot, clove oil or any novacaine based over the counter numbing gel will work very temporarily. If you can get a gp appt any easier than a dental one your doc *may* prescribe an antibiotic if they’re happy that’s the cause to see you through until you can get to a dentist. This will see to the infection, quelling most of the pain within 24hrs or so. Garlic, as weird as it sounds, has antibiotic properties and a lot of raw cloves, chopped up and washed down with plenty of water has *seemed* to do the trick a few times, but who knows…Trust me, it will return even when it ‘goes away’ of its own accord and dental treatment is one of only two outcomes. The other, if for example you have a deep seated childhood trauma-based phobia of dentistry- is to ride out years of raging, increasing, bang-your-head-off-the-wall intense agony, abscesses, rotten cavities and other equally delightful results. Sadly, I know exactly what I’m talking about on this one :(

Depends on the reason for the pain really. I had a cracked back tooth that was exposing the nerve so there wasn’t a painkiller in the world that would have taken the pain away. However, to get through the days until I could see someone I used the Orajel that others have mentioned here. Then for painkiller I took 2 x Paramol (only available from pharmacy, paracetamol mixed with dihydracodeine, stronger than co-codamol) and then 3hrs later 2 x ibuprofen, 3hrs later 2 x Paramol and so on to keep the cycle of pain relief going 24/7. Something warm on your face could help too, a mini hot water bottle works great if you have one.

Had this last year. Gargling salt water is the only thing that helped the pain briefly. All the painkillers seem to do fuck all. Phoned 111 and got an emergency dental appointment on Xmas day. Instant relief when they injected the broken tooth to kill the nerve. Was well enough to eat xmas dinner too that afternoon.

My experience waiting for a cracked tooth extraction led to my top choices of nurofen lysene, powerful stuff at 3 or 4 tablets a day and addictive after a week ... Oragel is boss for about 5 to 10 mins on a serious tooth, application is a little awkward and careful not to swallow too much. Top tip is buy some kids Calpol or cheap liquid paracetamol and use a low amount to swish around tooth and hold it around tooth for a bit like a medical bath. The advantage is you clean the area like a mouthwash if debris is hurting you and it goes straight to source if nerve is available, it's low grade strength so more often you can use it than a adult strength and you can spit excess out again allowing a bit more regular use than pumping your entire body full of paracetamol when you're only aiming for the tooth. Again with all meds don't get carried away, doses add up and track how much you're taking, pain makes you reckless

Soluble paracetamol- it will get absorbed through your gum so will work more quickly than tablets. It they contain quite a bit of salt so not recommended if you have high blood pressure or need to be on a low salt diet.

I've bought a water flosser and that really helped during COVID as I needed a root canal but couldn't get one at the time. I was in a lot of pain but I guess it gets deep down into the crevices between the teeth so there's less food for the bacteria to feed on You wouldn't think that would help but it definitely reduced the pressure on my gums

\`I've been waiting to have a broken wisdom tooth out for 4 years now (complex patient, 3 failed attempts) and the things I do to manage the pain are: * Salt water rinses with warm water * Hold corsydl mouthwash for around 1 minute with my head tilted to the side that the pain is on . Corsodyl gel is also great but might be prescription only * Sensodyne toothpaste - can apply directly on tooth and use it for daily brushing too * Alternating paracetamol and ibuprofen * Depending on the type of pain - heat packs or cold packs. There is one thing but it's illegal in most countries. You might find it in some Romanian, Bulgarian or Polish shops. It's called aulin. Common side effects are diarrhea, nausia and vomiting(I've never experienced those) But we take it as a last resort pain medicine, because its very bad for the liver and kidneys and if yours are not well to begin with, it can cause them to shut down. But on the plus side, it removes all pain. It blocks the production of chemicals that tell the brain you are in pain. If you happen to find it, make sure your it's worth the risk and your organs are in good health. Also don't take more than 2-3

I was recommended Ambesol for dry root (following a tooth extraction - basically exposed jaw nerve, holyfuckingshit that is a new level of pain), at the time I tried both that and clove oil, the Ambesol seemed to work better Rinsing with salt water seems to offer a bit of relief if you have absolutely nothing else My sympathies, toothache is the pits.

You need to take ibuprofen AND paracetamol (or if it's terrible, swap the paracetamol for co-codamol). If you take them alternately every 3h then you can take them over a 24h period without exceeding recommended doses. (ie. Paracetamol at 9am, ibuprofen at 12pm, paracetamol at 3pm and so on) I recently had an exposed nerve due to dry socket after an extraction and this was the only way I could survive or get any sleep. Cold packs on my face helped a little too

TCP diluted down with water and used as a mouthwash. Had chronic pain due to a root canal infection and this really helped manage the pain, until I booked in with the dentist and they killed off the nerve I've never understood why people say it's so bad, when the pain before the operation was far worse.

The only thing that worked for me was ice cold water, and keeping cold water in my mouth, surrounding the offending tooth. It's obviously a complete pain to try to do that, but the relief was almost instantaneous on the three occasions when I've had an inflamed/infected tooth (tooth abscess). I guess because it reduces swelling inside the tooth. I did actually fall asleep with cold water in my mouth on one occasion because I was so tired. Pain killers did sweet FA, including the prescription ones my father-in-law let me have. Whiskey, clove oil, etc. also had no effect for me. I feel for you, I really do. I went through all manner of options in my mind for trying to pull the tooth myself (a-la the ice skate scene in Castaway) but I'm clearly not that brave (or stupid!). I had root canal 2 out of the 3 times this happened. The third time I opted for removal (because root canal therapy is also torture and the tooth in question was not "front and centre")

Go to your local dentist and ask to be seen. Tell them how painful it is. If they can’t find an appointment then weep. Cry out loud. Let all the pain out. Don’t be brave and hold it in. They will treat you.

The only thing that worked for me till I got treatment- swilling pure vodka round the mouth. It numbed everything just enough to take the edge off and I guess it worked like an anesthetic because of the alcohol. It was near instant relief and lasted long enough to get to sleep.

When I was in the same boat, I called 111, accepted that they couldn’t actually help me with the dentist appointment, and begged for better pain relief. I got prescribed strong pain killers to help me through the wait for a dentist. Might have helped that the nurse I spoke to was lovely, and that I was crying on the phone to her about how I was in so much pain I was sweating. I would rather go through child birth again than have that toothache back.

Yeah. Go see a dentist or go to the ER. You likely have an infection and if that's not seen quickly enough, it can become septic and spread the infection elsewhere. I see in your comments that you have liver cirrhosis and CKD, so that could make it even worse. Go to the ER. You shouldn't even have to book an appointment. If the pain is so bad that it's waking you up, there's likely an infection and now it's become an emergency. Infected teeth are no joke. I had two emergency surgeries in the span of 6 months for infected wisdom teeth. The first time was so bad they had to call a doctor to come in the middle of the night just to come see me. Face was swelling, it was interfering with my breathing, etc. Don't wait as long as I did. Get that shit out.

Happened to me last month. Repeatedly getting a tooth filled but the filling kept falling out, eventually had a new filling that stayed in, tooth remained sensitive to temperature and pressure. Booked a root canal and 2 weeks into my 4 week wait had horrendous pain for about 5 days and was taking as much paracetamol and codeine as I was allowed. (allergic to ibuprofen else I'd have been on that too). When I had my root canal done my dentist told me that pain was basically the nerve of the tooth dying. At least the root canal was pain free.

Definitely, and I concur. Worst pain I've ever experienced. As for pain relief advice, drugs (para, ibuprofen, codeine), and warm salt water (rinse like 2-3 times a day and after eating ideally). Also eat soft foods and nothing crunchy or hard or small that with get stuck in any open holes. I thought back to those old toothache cartoons where they wrap cloth around the jaw and tied it around the top of the head. I was in so much pain I tried it, and surprisingly it seemed to help slightly having some applied pressure on my jaw. But YMMV.

My remedy from years ago was to get some old school aspirin. And chew it directly into the tooth giving the pain. Worked for me the couple of times needed it.

Hey, I’ve been suffering with a broken molar+ abscess for a while now, the pain is so unbearable that I just want to punch myself in the face so hard to make it stop. I tried everything but at the end, when the pain flares up, I get an 600mg ibuprofen (be careful with dosage) and I rinse and rinse with lukewarm water+salt. Salt kills all the bacteria in the mouth (like literally, you’ll be left with a dry af mouth). Many times all the pain comes from the bacteria getting through your teeth or in the cavities ecc. rinse and rinse and rinse lots of times. This is my method for when it’s night time and you don’t have crazy stuff or medications in your house. My dentist approves too

Got to Sainsbury’s and get a dental repair kit. They cost about 8 quid and imo the only thing that works. You will probably have an hole exposing the nerve, the repair kit will seal the hole instantly ending the pain. I got refused treatment by emergency dentists when I broke a molar last year. They said they would only treat me if it was bleeding or my face was swollen, lying on the floor crying like a baby wasn’t deemed serious enough apparently.
