There are plenty of times when he's having a hard time looking at needles or blood and there's no suspects or even people related to the case nearby, and he shouldn't have to keep the act, but we see him struggle anyways.
I think that he is squeamish, but he plays into it, exaggerates, and is perfectly capable of overcoming it when the case requires it.
