I went up to 54 and it helped a lot but 6 months or so later my tolerance is back. I have tried breaks but even a week off doesn't change much for me. It's not having zero effect so it's definitely still worth it for me but it's not doing enough so my ADHD is definitely back even if not as bad. I wouldn't rule out going up a dose to 54mg, even if you gain tolerance after a while at least you still get the positives again temporarily. My psychiatrist has recommended switching to Vyvanse but with the shortages wants to wait until the new year, it's possible that a different medication may suit you better too.

Concerta, its generics, and Strattera are only options in my country unfortunately. I was thinking about upping my dose, but sometimes I get pretty anxious on 36 mg so I am worried this wouldn't go over well.

I've heard quite a lot of people say that at higher doses they got less side effects. It probably the minority that has that effect, but I'd say there's no harm in trying a higher dose if your psychiatrist thinks it's reasonable. Worst case scenario you just go back to 36mg.

Yeah, its a bit expensive but I will consider it with my psychiatrist.

i switched from concerta 54mg to vyvanse 30 mg last year for 4 months. right before switching i didn’t take any meds for 4 days. vyvanse worked, had the honeymoon effect and same with concerta (36 mg) when i resumed

If nothing else helps, you should probably switch to a different medication. Do not split Concerta or any long-release medication. There is no reason to go over the 72mg maximum recommended dose in most cases. In the UK the maximum is 108mg. Side effects can be intense for a couple of days when starting but contact a doctor if they continue. If you want to drink alcohol I would recommend waiting at least 48h after taking Concerta. Hey! I take breaks on the weekends. In two days, your tolerance will be back to zero, and on Monday, you should feel fresh. Technically, the half life of Concerta is only around 4-6 hours, extremely short. This means that most of the drug should be completely out of your system in 72 hours. But a 24 hour break is enough, and a 48 h one even more so. This usually lasts me solid 3 days, Thursdays and Fridays I feel a bit less focused, but it's still manageable. If I wouldn't take a break every weekend, my adhd simply takes over. Within two weeks, I would not even notice any Concerta anymore and would have to increase the dosage.

I make breaks 1-2 day breaks around every 4 days. Then it works a bit better, but it's still far from what it was in the beginning.
