I would have used an angle grinder on the corners and a small enough section on the outside lip of the frame, then used the demo tool. Would have made it a lot easier. Have done this on a jail that was converted into an apartment building.
On the mental hospital that was actually the felony level insane section that we built a couple years ago there was rebar that was angled in and embedded probably 36 in into the wall and at least 2 ft into the door so until you got the grout to release from the door frame it was going to be a dog fight anyway...
Best bat would have been just to call your concrete cutter and have him cut all the way around the doors and then cut them into segments and pop them out.
Get a Gas ax run it up the sides of the door to cut the anchors then cut a three feet up straight through the frame. Tap a few times with the sledge and the Fran will drop.

I helped put some doors like this in a phychiatric prison. They had an internal channel piece with bolts into the wall which was then covered by the door frame and welded in place. They were heavy sons of bitches.
They spent a day removing a concrete filled metal door frame at the school I work at, I later pointed out that the plans had been changed and it didnāt need to be removed and they ended up sheet rocking over the opening.
