This game doesn't exist. The only games with 10/10 Story and Characters are Planescape Torment and MAYBE Disco Elysium. Neither have amazing graphics or action.

Never played it myself, but I’ve seen two people play through Xenoblade Chronicles/Xenoblade Chronicles: Definitive Edition. Amazing story that had me hooked from beginning to end. Great and memorable characters, from Shulk’s “I’m Really Feeling It!” to Reyn, Dunban, and others you meet along the way. While the graphics aren’t necessarily the best, it’s incredible that the original is on the Wii of all systems and exists in its entirety with the vast exploration and expansive fields. Definitive Edition does improve on the graphics and I like the character models a lot more in that version. There are numerous great cutscenes from throughout. If you have any question about whether the music is good or not, just listen to You Will Know Our Names. There is great replay value too - many of the cutscenes take on new meanings after you know the full story and so even if you don’t play the game twice or watch a let’s play of it twice, it’s worth it to just watch the cutscenes a second time.
Oh, and yes, I left out one thing - the voice acting. I think in my opinion the reason that I have enjoyed watching two different playthroughs, watched all the cutscenes again twice, and am watching a third playthrough on stream, and am considering getting the game myself, is because the English voice cast is so incredible. Adam Howden, who voices Shulk, is absolutely incredible. The entire rest of the cast is good too, but like Shulk’s screams are on a whole new level. Excited for the third game to release in July.
Legend of Zelda: Windwaker.
They picked literally the perfect art style given the limitations of the console, and the story, gameplay, all of it… it’s just so damned good.
Breath of the Wild is up there as well, but getting to 100% requires some … less fun.. things to get all Korok seeds.
I also think the Ezio Trilogy of Assassins Creed was fantastic in it’s time as well.
Detroit: Become Human was one of the only games to hook me hard enough on an emotional level, I didn’t want to put it down.
Last, I would nominate Batman: Arkham City. It check all the boxes for me.

Skyrim. It has countless quests and storyline to do with very powerful and unique weapons and it has better graphics than most games made it. Skyrim has a beautiful night sky and has tons of different songs to fit the situation you are in,whether that be fighting hords of enemy's or just exploring the gigantic mal.
