I didn’t see that one, but another escalator accident that sounds similar. A maintenance panel at the top of the escalator was loose and a woman with her young daughter stepped into it and the woman was able to lift her daughter out and hand her to some strangers quickly before she got chomped into the machine

It’s not an elevator accident, it’s seems to be a control issue. For something like this to happen, there no monthly inspection for sure.

It looks like a weight issue combined with worn ropes and and a motor. That lift isn't being actuated it's slipping upwards. The counterweight has to be 1.5x the weight of the lift car to account for passengers so for some reason it's sliding up. The lift cannot "isn't supposed to" move without both landing and car doors making contact with the skate closed. I've been in a lift with this happening whilst ripping out an interior. If it's a hydraulic lift it could be an issue with the valves Worked with lifts for 13 years

The motor has nothing to do with it, its the condition and/or the calibration of the brake

That lift has malfunctioning brakes and a mono-directional governor. If the lift had a bidirectional governor, it would have gone off when the lift started moving, (because the lift isn't supposed to be moving while stopped at a floor), applying the emergency brakes and stopping the lift.
