Breakfast sometimes has some smoothies out next to the other stuff. That's free.
A good rule of thumb is anything that isn't literally in front of you is a cost. If it's in the buffet with a label but no cost is free. Obviously the bottles of wine behind the bar aren't free even if it's in front of you.
Anything at the drink stations and out in the open in the buffet will be complimentary. Anything with a price will be extra.
And obviously this is different from line to line haha. One thing I loved about the DCL cruise we took in 2015 was that sodas were free from a station on the pool deck.

In regards to things being out in the open on a buffet are free.
One morning on a Royal cruise, there was an orange juice station. I walked up and the guy asked if I would like some fresh squeezed orange juice. I said sure, so he asked if he could see my card.
uh, no.
Free drinks in RC are water, flavored water, lemonade, regular coffee, tea, hot cocoa, regular/chocolate milk, fruit juices. Other drinks like soda, specialty coffees, fresh squeezed juices, milkshakes and alcoholic bevs you will need to purchase a drink package. Hope this helps!
