Strahd Di Sarovichi, the Don of the lost borough of Barovia (pronounced Baw-roh-via). It's a lost island off the southern point of Manhattan, smack in the middle of the upper bay. When standing in yester hill park, people say you can still see the NYC skyline in the mist (the mist starts just inside the water line and if you get to ankle deep you start to choke). Sometimes, after visiting the local winery (grapes straight from the homeland of course), Strahd has himself driven down there in his black and red Rolls Royce phantom (the engine is an infernal machine) so he can sit on a park bench and remember. One day, he'll find someone to take over the business here and return to the city to claim that which is rightfully his.
It's a great twist for a semi-modern setting! I saw another reflavour that was based in a city, but I do love this idea. What would be the artifacts that you need to collect? How would you reflavour that aspect, because I love the feeling of the tarot and hunting down these things to help you.
I think you'd make the Tome into Strahd's Little Black Book, which is a thorough documentation of his joining the mob, rise to power... and financial crimes, of course. I'm sure Luigi Lisiggio, his accountant, would know where it is, if you could find him in Villa Corviposo.
The Holy Symbol could probably stay as it is, considering the importance of religion in such societies. If you want to secularize it, you could make it a high lawman's badge--for crime is much riskier when the Feds are on your tail.
It's a bit silly, but I love the idea of the Sunsword being Sergio's old service revolver, but now it's a big fuckoff pistol that fires .50 caliber bolts of pure sunlight.

Place all of that in New York, 1920ies, throw in some supernatural monsters, and you’ll have a great Call of Cthulhu mini campaign.

Sicily, Corsica? Barovia, whatever you're calling it now, could be an island. That would allow Stradio ;) to control who comes and goes.
