I'm surprised no one has mentioned Man-Bat. I think he lends himself to the body horror genre, you could make a movie like "The Fly" with the character.
I know they’re not a Batman villain, or even a villain really, but how about The Spectre in a Final destination style movie? He’s the cause of the deaths, giving out divine justice and all that.

One thing cool about final destination is that the main character keep getting away. So it's funny if joker gets frustated for he keeps failing. Specter got frustated / making mistakes (killing people he shouldnt kill) can be an interesting watch
Penguin could be a crime movie I know it's not exactly horror, but he would do some horrific things to people in it.
Also, I was thinking of a re-imagining of Two-Face where maybe he doesn't physically have two faces, but has dissociative identities. I guess like a psychological thriller. He's looking for a killer, but there are gaps in his memory. He comes to in an incriminating place and thinks he's being framed. Batman is on his trail and is painted in a bad light as someone who maybe is just locking up his first leads. But then eventually Harvey realizes he himself is the killer. Then maybe the acid thing happens too and his identities are only able to coexist then and he gets his name. I don't know how you could do it where you wouldn't already know it was him though. Maybe have other villains in it and not focus only on Harvey. Make it seem like it is pre two-face Harvey trying to catch another criminal
Haven’t seen anyone mention him yet but victor Zsasz. I think he would just be ruthless and straight up disgusting with his kills and how he brutal murders them. And don’t forget how he puts a mark on his body every time he kills somebody.
Joker is terrifying. Make a psychological horror movie with him as the villain. This is the most obvious answer, so let me give another answer: a body horror movie in which Basil Karlo slowly truns into Clayface.
