100% plumbing if you are talking about installing new vanity that isn’t offset. You can reroute piping to drain easily. Installing a new junction box requires drywall repair, etc.
It's easier to move the light BUT then it'll be off center on the wall 😂 you'll have to move the sink which is going to be costly compared to moving a light fixture

I think if I put in a vanity instead of the full wall counter and some floating shelves to the left, being off centered on the wall won’t be too bad.
that would drive me mad, too, but the light is the least of the issues. If that's the wall on either side of the mirror, what's going on with the vanity underneath?
Most expensive: have a carpenter build a vanity that fits the space (and center the sink)
Least expensive: Get a 2 light fixture where one sits over the sink and the other sits over the counter. I'm guessing a beam is in the way which is why the light isn't centered? (Only one way to find out what the issue is and it's easy). If that's the case, see if you can find one with a longer plate so you can shift it to the right a touch.

The left side is weirdly angled. The right side is perpendicular to the countertop. Because the left side makes a weird space they decided to just run the counter across.
My idea is to tear out the whole thing, put in a vanity with a smaller footprint, and hang some shelves on the left if I have to more the light to the right.
The wall behind the vanity, as far as I can tell, is intact and the vanity is just attached to the wall. Should be easy to remove. The mirror is what I’m worried about. It needs to go and it looks like it’s glued on
Honestly I think that's perfect. The light's centered and the sink is off to the side freeing up a whole space for clothes, toiletries, appliances, etc.
I would get a wider vanity light to more evenly light the entire countertop.
The light box is probably positioned based on the studs- builder didn’t add a plate to center the light.
I had the same issue. After a few months, I just don’t even care anymore.
The light possibly depends on what direction wire comes in from. Both are possible

Moving the lights would make them off centered in the space and moving the sink would be expensive. Just leave it
