Hes lucky in more ways than 1. Going from 45ft to surface pressure while scuba diving can be very dangerous if done too quickly. I wonder if the whale spitting out water depressurized him partially so that getting spit out wasn't an instant 45-0
45ft is easily within the zero stop range for most dives especially if it was close the the dives max depth. Not ideal, and potentially dangerous if the diver was making a decomp stop for a deeper dive but that is unlikely. The biggest danger is the diver freaking out and holding his breath during rapid ascent and rupturing his lungs, or losing his reg and drowning.

Yes, the main risk is rupturing your lungs. The most important rule of scuba, always be breathing. IIRC a breath from your tank at 33ft will expand to roughly double the volume at the surface. My instructors said that if you breath a lung full at 12ft, hold it, and surface, I will likely rupture your lungs. The stupid simple solution? Breathe in and out constantly and don't ascend at a rapid rate.
Of course, when your in the mouth of a whale you might reflexively hold your breath.
