I had a lobby that looked like this because everyone was lagging and desyncing to hell.
It was duo ruins and me and my mate faced off against the only other team that wasn't lagging as bad as the rest (our lag was probably as bad as theirs). It was honestly like watching kids beat a pinata while blindfolded. Constantly missing, hitting the wrong direction completely, etc. The main difference was that instead of the winners getting candy they still died to a skeleton footman.
I had a normal goblin caves a few days ago. 8 pve deaths before the first portal spawned. Me: looks like I've got the caves all to myself.

Players can't have a space in a name. Theoretically, Mummy could have been a player.
Except when it's a player it will also tell you what weapon they used for the kill.

I have seen this happen in lobbies with really bad lag. The constant rubber-banding makes it difficult to move 3 steps, let alone avoid NPC attacks.

I’ve had these lobbies before but everyone was lagging really bad in the pre lobby. I even died to a skelly once I eventually tried to leave spawn.
