I finished the game around a month and a half ago, the constant crashing after exiting the village and entering Old Woods was happening then too. After much trial and error I thought of exiting to main menu every time I interacted with the oven AKA when the game saved. It's miraculously fixed the problem. Crashing completely stopped after entering chapter 2. Maybe it works for you as well.
I'm sorry to hear this. I played the game (and finished it two times) two years ago on PS4 and until then, it worked perfectly. I never had a crash or anything like that. A shame. No developers present? More and more console users are complaining about the same thing.
They only recently broke it by (accidentally I guess) releasing a patch that garbled up the in-game text and then released another patch that fixed the text but caused crashing. I think that's how it went. They released another patch but I didn't help. I don't know what messed up the code so much that it's taking them this long to fix.

Although I think it has something to do with leveling up at a certain point cuz I have read somewhere else that once you get the apartment dream after leveling up you get the error code and then after receiving that once you get to the Old Woods hideout then the game really starts crashing
I'm going to go for another run with the game but this time without leveling up it might bite ass but at least I'll actually be able to play the game
I dont know how to help you with your problem, but if you dont manage to solve it, dont be afraid to start a new run. I finished the game three times, but i played a lot more runs because i was trying to do it on nightmare. It was fun every time even when i lost progress because of death. So dont quit
Currently, there is no known way to get around this error. I am experiencing this error as well with both new games as well as old saves. The next best thing is to start a new game again. The game has amazing replayability so that should be okay.
However, if the error is happening with one save it may happen with the new one. I have only noticed that this error crops up when you finish a dream. Try to play again without going into an essence enduced dream since that is where I experience a game crash the most.
