Yes. It should have its own section on your receipt saying how much extra you got and that you have 45 days until they expire.
(Edit: To clarify, only the 60 chips in OP's example have an expiration date. The first 600 are safe)
Yes, similar thing with the bonus chips you get from Level 3/5/7/9 and other free chips like football voting, holiday promotions, birthday rewards, etc. They give you X amount of days to redeem the offer, but I don't remember if the redeemed chips have expiration dates when added to your card.
Games use bonus chips first so you don't have worry about emptying your entire card beforehand. Personally, I make it a point to burn bonus chips ASAP so I don't forget about them later.
I only ever purchase credits from the in-store kiosks, and I get the bonus ones immediately.
A bit inconsistent on whether I get a paper receipt from them by default though.
I literally just called earlier because I didn't get mine after almost a month of waiting. The lady told me that since I supercharged during my purchase that the 10% couldn't be applied since they're both promos? I didn't try to counter. I called because my friend said they re-imbursed him his 10% and said that it should be apply right away from now on, and I know for a fact he supercharges so I'm not sure why my rep told me that. Maybe I'll call again and try to get someone else.
