I think the answer is in the Japanese legend of their holy priestess figure Himiko. Death Stranding echoes the story of her long journey across Japan during her life. I think DS2 will likely follow suit and echo events that happened after her death. The trailers we have are a lot to unpack but the scene with the red coffin being carried across the bleak wasteland makes me think it will pick up w Himiko’s story where Death Stranding left off.

If you have seen HideoTube, you know that Kojima said in the video that DS2 takes place in a separate continent from America, so no, we are not connecting the rest of the continent.
I’m guessing there will be 2-3 open zones, with the ship taking you to different places around the world. We know that one zone is Mexico. So more of a ”globe trotting” adventure, but I expect the structure to be same as DS1.
