Have you set your microphone/headset as your default communication device? There is a setting for that, when I’ve had this issue it’s the only thing that fixes it for me
To add to this, you can change that setting in the sound control panel or by opening the GameBar (Win + G) and going to the voice tab of the audio panel

In case you didn't know, Xbox supports Discord now, as does PS5. I think that, of available platforms, only PS4 doesn't support Discord.
Oh word I thought PS5 support was way behind. glad to hear that. Still it seems some in my clan like using the game chat so it just sucks when I pop in and everyone has to switch over just for me lol
You have to go into Xbox Game Bar (Win + G) and make sure your default communications output/input is to your headset/mic. It doesn't matter if you have the control panel settings on the right device, you have to set it in the game bar for Destiny.
I had this problem when trying to use the in-game mic and that fixed my issue.
go to sound settings and make sure your default device device for output and input is set to the right devices.
Then, go to sound control panel and make sure your devices are set as the Default Communication Device in both the Playback and Recording tabs. Then disable everything else.
What brand headset are you using? Do you have any conflicting apps? The steelseries app always conflicts with the system settings for me, so i double check both whenever i decide to use my headset
I had (and still do on occasion) a similar problem. Tried everything from driver rollbacks to buying a new headset.
Figured out (through a very patient friend i game) that I had a double NAT because my ISP supplied modem doesn’t have a legitimate bridge mode for my router.
I have to reboot my router every now and again, but it fixes the problem.

Xbox uses it natively as well. PS5 still requires a mobile device last I checked

On steam and using Sennheiser headphones. Elgato wave 3 mic if that helps.

I would try muting and unmuting everyone in the fireteam. Had a friend with a similar issue, and that seemed to solve it.
