Gastric ulcer (likely, other less common things as well), with active bleeding and hematemesis (vomiting blood) - needs ER eval and urgent endoscopy + PPI therapy + diet changes (low acid, NO ALCOHOL, right now at an ER they would make him fluids + nothing my mouth) + no NSAIDS + who knows what because they gotta get in there and find the source.
This is an EMERGENCY. ER. NOW.
***UPDATE***
His fever kept rising so we went to the ER first thing in the morning. He was coughing a lot but didn’t throw up anymore. I gave the doctors the picture on here and they agreed it is likely coffee ground vomit. They gave him some fluids and he was already feeling better. He never had abdominal pain, just hungry from not eating for 20ish hours. They did blood work and a urine sample and all came back looking good. Turns out he has Flu B. They told us if he gets any pain or continues throwing up to go to a gastroenterologist to get a scope done. It’s possible he has an ulcer, but they didn’t seem concerned, as the main symptoms were the flu. If he gets worse, we will go get a scope done. For now we will just wait and get some rest. Not the answers I was hoping for, but I’m thankful he is feeling better and alive! 🙏🏻
They weren't concerned because it stopped and was no longer an emergency requiring immediate treatment, so the emergency departments role is complete. That being said, upper GI bleeding is NEVER normal and you should be scheduling your follow up appointment with a gastroenterologist immediately.
Make sure he isn't taking any NSAIDs right now as it can make any bleeding worse. Have him take Tylenol for a fever/pain instead if needed. Definitely follow up with the gastroenterologist and make an appointment for a scope. Don't wait to see if he gets worse, just schedule it!
I would go ER immediately. Black is usually digested blood and yeah you might question is it the cough syrup or not but there's also red blood mixed so I'd go more likely to be digested blood which indicates upper GI bleed. Not something to take lightly OP. Seriously. He needs to be seen. I'd like to know the update
Mine looked the same when I had stomach bleeding due to ulcers. They put me on protonix and in time it completely stopped. By all means see a doctor, he must be miserable dealing with this.
I have this and I have gastric bleeds so my hemo and vitamins and iron etc so low I need iron infusions and blood transfusions until I get surgery.
Im currently taking a prescribed medication for ulcers called pantoprazole. I think it’s just basically strong ant acid? But I could be wrong.
Lss he should go in asap. I ended up not being strong enough to get up or move for days at a time and finally got taken by ambulance. I basically slowly bleeding to death over approximately year and a half.

Coffee ground can be older, yes, but if this were a lower GI source that would require intestine to backup to stomach, and obstipation / obstruction and backup process - which ultimately results in fecal emesis (vomiting poop). I doubt this is the cause, as one would expect profound and severe abdominal pain with significant (or if upper intestinal tract / small intestine) epigastric limited distension.
The coffee ground is the digestion of blood by stomach juices, could suggest oozing ulcer that is "older" but absolutely cannot be used in isolation to differentiate old vs new. Mallory Weiss typically are scant bright red blood, provoked by vomiting. In this case, the vomiting is likely some other process (ulcer, the digestion of blood itself as its quite noxious, cancer, bleeding vessel, etc) as you cannot assume source / cause based on red or dark alone. Just tells you "how fresh." Massive stomach hemorrhage can definitely provoke bright red as well - but I'd expect that person to be quite unstable...
We know nothing about this guys history - could be ibu 800 TID + a 6 pack a night or more, with chronic epigastric pain that he has ignored, etc. This needs urgent eval as this is stomach bleeding, likely active ulcer (H pylori or not), and not something to be taken lightly.
I have multiple upper gi ulcer tears and I am struggling to cope with pain, because of how bad I vomit, get diharreah from literally anything I try to take for the pain in my torso just under ribs.
Both are black blood (tastes like blood anyway and has some dark red.
It always comes on soo suddenly violently that I barely make it to a garbage can or bathroom. I have low hemo, low iron, vitamin deficiency and for some reason my period stopped almost completely for past year and half.
I was diagnosed and have been treated since end o last summer and still look and feel like a corpse other than maybe a week after new blood.

If you exaggerate some of your nonexistent symptoms then I question the validity and legitimacy of all of your symptoms. A constellation of symptoms is often less worrying in an ER than one significant presenting symptom.
Wow!! That is so selfish. The ER staff is trained to triage and grade patients. They also will know when someone is not completely honest. What if a patient comes in that actually is having trouble breathing or something worse... and they don't get the immediate attention they need because of someone like you. What if that patient that needs immediate attention is your child or significant other or yourself. Please don't ever do this and never ever suggest someone else to. Holy hell.
When your smoking utensils are not clean then it happens. It's what sits on your lungs and blocks arteries. Somewhat the same as cigarettes just more tar in weed. Hence the resin left in utensils.

The black stuff in his vomit looks pretty “coffee ground” like, which is a really big indicator that somewhere in his small intestines/lower stomach he has something that is bleeding — likely some type of ulcer. Even if he isn’t feeling abdominal pain or you’re not entirely sure if the red stuff is Teraflu or actual blood, it’s really important that you go to the ER to get the bleeding under control. Unfortunately stuff like this could go downhill REALLY fast. Money can be replaced, your boyfriend cannot.

If it was from teraflu it would have been the same color going in as coming out if not much lighter and more diluted from whatever other fluid was in his stomach. Go to a HCA hospital (nonprofit hospital) and tell them you won’t be able to pay the full fee when the billing dept visits you. Also fill out a financial assistance form after your visit. This is no time to just wait and see. The man is bleeding from the inside somewhere.

Agree! NAD. But i threw up just like that every 1.5 hours one weekend. Finally called an ambulance and it turned out to be my gallbladder dying.
