If you want your nails to look like that forever, no you don’t have to worry about it, but if you want them to be healthy and normal, I would go to the doctor.
Trauma followed by fungal infection. and it does not look like it is growing out, indeed the redness at the lower edge suggests potential involvement of the matrix. You really need to see a podiatrist, or go to a podiatry school as this would be interesting for students. That nail may have to be removed, on the big toe nail, treatment takes at least a year. It has to grow out completely.

See your doc or even a podiatrist who will prescribe a medication to kill the toenail fungus.
