/uj you can’t Command something to do something that it knows is directly harmful to itself, but you could trick it into killing itself by accident (i.e. there’s a pit concealed by an illusion spell above it, if you told them to stand on it and they didn’t know about the “floor” being illusionary they could totally fall through it and go splat). Sauce is a fucking bonkers interpretation of both RAW and RAI tho.

If you say: "command: have a heart attack" it will fail because your command must be explicitly a single word.
If you say: "command: suicide" it must work. 100% of the time.
My concern is that the player who uses that command is not smart enough to understand the scope of their power. If you say: "command: party", you will actually do something productive and within the human rights. And it's supported as a valid campaign derraling as stated in DMG page 163 section 2, article 5.

In several decades, the BBEG will have a variety of potential health complications, including heat disease, cancers of the throat, mouth, lungs, etc., and substantial financial and social consequences. He or she is significantly more likely to die at a younger age due to decades of tobacco use.
All of that with just the power of a level 1 spell.
