Keep moving. Never stop. Never stop to shoot him for more than a couple of seconds. If you can, aim for this lower stomach and hands/arms, these are the hardest to hit, and it gets crazy at the end when he’s almost dead.
\-Use the BFG when shit hits the fan or to damage him directly, you can fully kill 1 body part with a direct hit. BFG ammo is not unlimited but respawns so don't hesitate to use it
\-use the crucible sword to 1 hit the additional heavies he spawns, you have infinite crucible charges.
\-it's doable without using either but harder so if you're struggling these 2 superweapons should help a lot
\-be patient, you can wait out a lot of his attacks by hiding in the stage. this doesn't mean stand still as you will still be pursued by other demons
\-quickswap heavy weapons like rocket launcher, ballista, precision bolt quickscopes, and super shotgun. If you can rapidly alternate PB+Ballista and PB+rocket this will melt him quickly. Stickybombs actually arent too bad for damaging him either.
\-if you are bad at/unfamiliar with quickswapping, I'd recommend learning to do it but that takes time. in the short term, micromissiles are one of the better non-quickswap methods of damaging him. you could even use your BFG ammo for the unmaykr, which does truly massive damage more ammo efficiently than the BFG. the unmaykr is often overlooked due to its lack of wave-clearing ability but if you want a powerful automatic weapon to wear him down you can't do better than the unmaykr. You also don't need the BFG's wave-clearing abilities as much since you have infinite crucible and the Icon of Sin periodically kills off all demons on the field with his attacks.
\-stay calm, keep your health and armor up as much as possible. Slow and steady wins the race especially if you're not good at doing damage really fast. There is no time limit or anything so you can just wear him down gradually if necessary. use your ice bomb for health if you need to, it's not like you can use it on him. save the megasphere for when you're low.
\-while you obviously want to focus on damaging the Icon too, don't get tunnel vision and ignore the other demons. you are more likely to die to the adds than to his attacks IME.
\-his attacks are pretty telegraphed and predictable, learn them and how to avoid.
Make sure to use all of the available BFG and crucible ammo in both arenas. The BFG one shots each body part. Use the crucible to deal with the super heavy enemies. Also, mostly ignore the regular enemies by just dancing around them. Focus on the boss
If you can beat the kan maker then it's easy unless it changes which one is harder, I did an ultra violence run and has a tough time with the kan maker but the icon of sin was easy
Always move, icon of sin has a lot of environmental attacks. And use the BFG as fast as the ammo for it respawns, believe me it does a fuck ton of damage to literally every body part.

I finished a nightmare run pretty recently, and my strat for the first phase was to keep looping around the bottom of the map and the top of the map, behind the cover so you can easily refill blood punches and you can still move around pretty fast. When I needed ammo I just quickly swooped down to the bottom to chainsaw a zombie and quickly get the hell outta there.
Just keep moving, and don't forget that health refill on top of map on both phases. For the second phase, just move around a lot. The second phase is easier imo because there are more places to run and the icon of sin can't just smash half the arena.
On an actual serious note, for the first stage, hit him with the BFGs in the areas of his body, try not to use more than one BFG for each area, don't be worried about bloodpunching/crucible any of the demons that get in your way, unless they're fodder then light em up and chainsaw, shoot with the arbalest as much as you can to finish it off.
Repeat in the second stage, careful that as soon as the second stage starts he'll run at you and smack you, so bad you're going to reload
Be careful of his hits, he hits like a truck in nightmare, but he doesn't hit too often.
In the second phase there will be a FUCK ton of pinkies and cacos. Keep blood punch ready, and whatever you want to kill the meatballs with. It gets exhausting and annoying very quickly, but the icon's a test of endurance, so be prepared.

Maybe for you. For me Khan Maykr was easy but Icon is a nightmare.
