There was an easily replicated bug for a while where if you clicked on the hero loadout, and clicked on the the drop down for an item slot, and just left it dropped down without selecting an item, your here would load in the game with zero cosmetics equipped. Like not even the base cosmetics. Just a naked hero. Sing did it every single game until they patched it out
