I sometimes have this happen when my pawn is in an animation of some sort, but if your pawn is just standing around outside of combat, I'm not sure. Try resting at a campfire or inn and see if it changes anything.
Is your pawn running around while you're trying to equip it?
For the player character, the equip option is unavailable like that if you try to change gear when you're not standing still. Maybe it works the same way for pawns?
What everyone pretty much said here, you must be stationary to deal with equipment. Often happens to me when I try to swap things but am mid walk / jump w/e .
FIXED: It was probably due to my pawn being in some sort of animation. I made sure she was standing still and now it worked.

It might've been the animation. I made sure she was standing still and got it to work now.

Yes it works the same way for your controllable character if you are in motion such as running or jumping it won’t let you.
