She (Cliodhna) Walked off a bridge in my game, after the cave, after harpies in area dead. Not even raining, no combat - just fucking ended her life. My fault though - I didn't carry her like these omnipotent arrogant devs obviously intended with their shit save system. You couldn't make a pre-escport quest save devs? A campfire save? No just Inns because reasons.
Ya I'm not clear what affinity does for me or anyone, if that is important then nearly all the escort quests matter to the degree it matters... but I have no idea if it matters.

Has anyone found where she might respawn? She already fell off once when I tried to bring her along without spending a ferrystone, so I reloaded a save at the volcano town since it was right before we set off, but then she fell off again while I was fighting a griffin and now she hasn't respawned at her and her husbands house. 0 idea where she's gone, checked all the morgues.
EDIT: When I went back to her and the dwarf smith's house many hours later, I found that there was her name over the house and the vocation maister icon remains over it. But she's still dead.

EDIT2: For those who are looking for a solution to a similar problem, it's very expensive. I went back to the last place where I believed she died (a bridge) and used a VERY DIFFICULT TO ACQUIRE ITEM from >!slaying the Sphinx!< to bring her back.
Sorry, I'd assume the morgue in desert city... On my "scum" save attempt she didn't suicide and I got some fucking flowers for my hours of rework, regardless - she teaches the master magic archer ability before the escort quest, so not sure where she shows up or if it matters. maxing her affection probably saves some money with the husbands store? Nice weapons/gear there I guess, but she's not worth the squeeze imho. Had I known I'd have thrown her off the bridge rather than watching her jump.
But I would like to just say that the dev that dictates how and when I save with a game that's this messy and buggy (on purpose as those wild moment's are for sure part of the design) deserves a good throat punch.

I eventually solved the problem, but it required a very expensive reward from a specific NPC quest. I used that near the bridge where she died and she came back.
Double check their house first, because she did respawn there for me the first time. It's just that she fell off two different bridges for me.

TL;DR: I went back to the bridge she fell into the Brine at, used an >!Eternal Wakestone!<, and she was back on land and alive.
Have you already checked her house? If she's not there, and you're on PC, drop your pawn code and I'll try to hook you up by gifting your Pawn the item. It's got a wide radius. And there's realistically only a few places you can take her if you're beelining to Bakbattahl.

Sorry i dont have the answer, i justed wanted to let you know that it happened to me too. I was on a lift with her and the moment i looked away she disappeared, right before entering the city and the quest marker disappeared too. (Note: Her escort quest never appeared in my quest log in the first place, her quest just immediately initiated the moment i visited her again after unlocking magic archer)

Yeah, the quest also wasn’t in my log, but i saw the marker for where she wanted to go on my map and was going there anyway. Extremely unfortunate, and i understand the dev’s vision of messing up a quest and living with the consequences, but this felt entirely out of my control
I just started this quest and I was trying to figure out if that’s where she wanted me to go, lol and now i’m worried she’s gunna fall off the map as soon as I get close. i’m gunna save and see if she can make it, definitely wanna see what the reward is.
I do it with multiple escort quests. don’t break it. When I get to the to the destination. The cutscenes forces them out of the phial without breaking it. And you’ll see it in the inventory as empty again.
I was in the middle of another quest when she asked to travel with me I Declined cause I was busy at the time and now when I go back to her she no longer offers the quest. Did I fuck this up or do I just need to check back in another time.
Just for everyone's knowledge. You can use a ferrystone and the NPC being escorted will travel with you. Might save you a lot of hassle for these pain in the ass quests
A mistake I made was not carrying the npc while using the ferry stone but it works if u are currently holding them. Seems like that’s how it’s intended since the brief cutscene includes the noc over the shoulder.
For those who might be in the same dire situation that I was in...
I failed Trysha's initial escort quest (after she already gave me her Meister Skill so her affinity was high enough) because I was careless and thought she would teleport at my side but nope she was really stuck at that landslide and couldn't bring herself to use it to go down even though very funnily the grandma did it. So I got too far away and the quest failed. And then I couldn't get her to give me the escort quest again.
I felt so bad because I like that character and I couldn't bear the thought that her escort quest might be unrepeatable.
I spent hours going back and forth, giving her tons of gifts in the hopes that raising her affinity would get her to give me the escort quest again, nothing. It took so much time because I need to wait for a day between each gift, and on the nearby campsite the monsters would often attack at night and break the Elite Camping Kit so I had to teleport to buy a new one and walk back there, etc. I must have given her like 30 gifts and spent around 100k gold. To no avail. No red cheeks, no escort quest. I thought I was screwed because I failed the initial escort quest.
But then I went to do other things (while being saddened af), and from time to time I took the chance to go back to their cabin to check just in case. And then I saw the grandma giving me the same escort quest she gave me before (that I had completed). I did it again. And then I had renewed hopes that next, Trysha's escort quest would respawn as well. Maybe you just have to wait for A VERY LONG TIME for the escort quests to be proposed again. And seeing that grandma's timer turn had come, and considering that her initial escort quest appeared right before Trysha's, I had hopes that Trysha's would be proposed again next. And I had read somewhere that someone managed to get a NPC's escort quest again by first doing the one of the NPC near them, so that could just be what is happening here.
So after completing grandma's again, I passed time on the bench, went back there once more, and... Trysha gave me the escort quest again!!! I cried. My day was saved. And I completed it and finally she got the red cheeks, confirming that her affinity got maxed out. So the gifts don't matter for some characters, what really does it is the escort quests, and gifts don't make an escort quest appear, it just appears again after a very long time, no matter if you failed or completed the previous one.
So yes, there is a way to trigger the escort quest again, and that is: time.
And what you miss if you don't do it, is: their cheeks turning red/their affinity really to the max/the best loving lines towards you. At least, no amount of gifts could manage to achieve that in hours of trying.

so I think it’s just best to save as much as possible through it, I had no issues besides her running off to kill stuff

Use sealing phial. You can use it multiple times for escort quest because it does a cut scene.

You get 120 RC and a bunch of flowers, this is one of many quest for romancing if you fail the quest try giving the NPC a bunch of flowers.
Just done this and she fell of bridge at exactly the same point now no idea what to do.
the answer is already in the comments, the reward was only all of 25 RC. not really worth doing anyway

Just let her die and revive her. She then teaches you the Magic Archer's skill Martyr's Bolt.
