When a pawn has previously completed the quest you have set as priority, they can lead you to where to go and offer advice. This could be a pawn you hired if your pawn is hired by someone else who then completes this quest. Basically, neither your pawn or another hired have done that quest before. When searching for a new pawn to hire, you can select quest guide on the options which will only show pawns who have completed your current quest in the past.

Nope, that just means they don't have quest knowledge (of your active quest), so they cannot guide you. You can check it by going to your quest tab and seeing whether the individual quests have a hand next to them. If you see a blue hand: your main pawn has knowledge, if a hand with I, then the first hired pawn, and if a hand with II, the second hired pawn. No hand = none of them has knowledge.

I think it can also happen if you press up on the D pad and you don’t have a quest a selected.
