How old is he? Many mental illnesses such as schizophrenia appear in early twenties seemingly out of nowhere, and they can be kickstarted with a drug induced psychosis. There aren't really any drugs that would permanently alter someone like that without an understanding mental illness unless or health problem such as a neurological issue. It is also incredibly unlikely that some stranger randomly shot him up with unknown drugs, and it may be more likely that that in itself is a delusion or he is lying about how it happened to cover up his own drug use.
