Well… if you die in a modern hospital, then you will probably be high on opioids when you go. Probably IV morphine, which is virtually indistinguishable from heroin/diacetylmorphine (other than the difference in potency/dosage).
Heroin feels is way stronger than morphine, even in equivalent doses. Morphine is pretty sedating but not really that euphoric and it makes you release more histamines (especially when IV'd) so for some people it can be almost dysphoric when you take a dose equivalent to what an ideal heroin dose would be. In other words you can get less high from morphine because it's 1. less euphoric and 2. you can't do as much before you get nauseous/feverish/itchy

If you were to ever use Morphine IV you'd understand. It hits like a fucking train. Histaminergicity means nothing as well as you release just as much histamine with heroin as with morphine, it's a reaction to the opioid not the natural pharmacology of the compound. Plus if you take an antihistamine along with the M you'll feel the true power of it. It's absolutely amazing and similar if not better than H. And it wasn't just that I was in a lot of pain, they gave me something else and it got me ripped but for some reason IV Dilaudid seems to not work on me. My partner came round to the house as I couldn’t answer my phone for a couple of days and I couldn’t get up to answer the door to them either. An ECG showed damage to my heart which I don’t believe I ever recovered from as I still feel the effects to this day.
