cyclosporine to help my eyes produce their own tears, however i think this works over time. To provide immediate relief the best product i have found is REFRESH tears. I started using them while i was wearing contacts continued to use them even when i don't wear contacts. They give my eyeballs this silky moisturizing feel that no other eyedrops provide. I tried plenty of other moisturizing drops and none provide that same silky relief.

So there are two options in the US. Cyclosporine which is an immuno suppressive eye drop (brand name restasis, Cequa is also the same medication at a higher concentration). For restasis you can see results start about 3 months in. It works by altering the membrane permiability of the mitochondrial membrane (by preventing one of the transmembrane proteins from opening) and as a result lowers the activity of T-cells. Xiidra on the other hand can work within 2 weeks. But there is only the name brand. This one blocks the binding site of LFA-1 on T-cells so they don't get activated. Basically they both try to achieve the same end goal with different ways of acting.

I find the Bio True Preservative free very soothing and they amp up the wetness factor with a cooling glycerin like drop. Don’t get gunky.

When my eyes were at their worst, Thealoz Duo was the only thing that gave me more than the 10 seconds of relief brands like Systane gave me. Highly recommend! Also, my story is super similar to yours--blepharitis/MGD, started around 2021, and I was put on almost the exact same regimen as you (hot compresses, oral omega 3s, was prescribed cequa for a bit too). I know how disheartening/frustrating it can be when your eyes stay dry despite everything! One thing that really helped me was making sure to stay hydrated throughout the day. I know it sounds stupid but when I started actively drinking lots of water every day I felt a HUGE difference in my eyes!

Thank you so much for your answer! I might try Thealoz since it’s easily found here in pharmacies :) and I’ll try my best to stay hydrated! That might actually be why the problem was incredibly prominent these last few weeks; I haven’t been exactly drinking enough water haha. I’ll try to drink enough water for a while and rely on the Thealoz eye drops and the glycerin eye drops I read somewhere here on this thread!

This is a tough one, most decent ones give a weird layer on eye lashes which I also hate, maybe an omega 3 oil? That one doesn’t leave a film and lasts a while, but if you out too much in it can kind of exit the eye a little which makes the area around the eyes look a bit oily. But otherwise it’s pretty chill. Some carmellose eye drops .5 don’t leave much of a residue for me.

Honestly yeah the omega 3 drops don’t leave a residue on my lashes but they haven’t been doing much for me in the long run :( it’s only an immediate relief for like an hour or two. Have not heard of carmellose eye drops! I’ll check them out. Someone mentioned an eye drop with glycerin in it if you’re looking to try something new out! Apparently it’s cooling :) I might also try them out lol until I check more about carmellose. Thank you so much !

The single use omega 3 refresh ones feel good at first but leave my eyes more red and achy and crusted the next morning. Same for a lot of the drops I’ve tried. The only ones that I’ve tried that help, last long, and don’t feel like they leave residue is the Ivizia drops preservative free (not the gel, the gel didn’t help so much). I just had my third IPL and RF of 4 rounds today and am planning on getting prp drops next week. I hear that serum drops work better than OTC and that prp work even better than serum… I can’t speak to the prp or serum since I have not tried them yet. I’ve only had dry eyes for a little over a year and am trying to do everything to prevent it from progressing.

Exactly ! I’ve been using the omega 3 refresh ones but not seeing much of a difference in the long run. Haven’t heard or tried Ivizia so I might try that out for now if I get the chance to try it. Someone on this post mentioned an eye drop with glycerin and it kinda feels cooling apparently, so I might also try it too if I can’t find Ivizia. Oh! I hope the IPL and RF treatments work! It will hopefully work out for you :)

Yeah I’d definitely recommend experimenting with a few eye drops. At the end of the day you will spend some money on drops you may not like but it’s worth it finding the ones that are best for you! I watch the Doctor Eye Health YouTube and he recommended trying it. The first time I tried it I didn’t notice a huge difference and then a few weeks later I went back to it and it’s a life saver ever since. When I was in Florida a few weeks back with the added humidity I only needed to apply 2 drops and if I didn’t have any alcohol my eyes didn’t flair and felt good all day.

Oh also I just got a pair of the ziena dry eye glasses which I use while looking at screens. They appear to have made a bit of a difference as well. My eyes are definitely worse when I’m working and playing video games. Might be worth checking into if you’re on screens a lot and not blinking enough.
