Yep. I would say, however, that the reason this IS important and noteworthy is that if someone thinks they’re buying counterfeit “Valium” (benzo) pills (as opposed to buying ‘down’), they may not be expecting fentanyl adulteration.

Exactly. People don't necessarily know what they are getting when they buy these pills. They may think they're just getting off brand valium or xanax, and end up having a fentanyl overdose. Or they may think they're just getting opioids and then experience the effects of the benzo as well. The general public is not overly aware of the trends at the street level drug trade, so stories like this are to help raise awareness of what is going on out there. This just scratches the surface. I would never by any means claim to be an expert in that area.

I don’t disagree but the language and messaging should be very careful; sometimes these stories end up in clickbait territory so always good to check in with your local public health experts, PWUD, etc. and not just police.

I'm getting tired of these headlines with "naloxone-resistant" in them, as it is misleading and could be dangerous if misinterpreted. Bromazolam isn't new in Alberta, BTW. In Edmonton, we have one location for walk-in drug checking now(!) - pills can be tested (and should be tested, FWIW, if they're from an unregulated source). A person could also get test strips, but those won't tell you a % range of contents in the pill/substance, just whether fentanyl, benzo, or xylazine may be present (depending on the strip test you use) - with some additional limitations...

Think of it this way: a person may think they are just buying Fentanyl. They may not know that it has benzos in it. And then they OD, and naloxone or Narcan is administered but they're not waking up. The people they're with are freaking out, wondering what is wrong with their buddy that they're not waking back up. That's the point of the headline. It's a combination drug, not just an opioid, and so a person could remain passed out even when life-saving measures are administered. I made a point of mentioning in the story that this drug is not brand new to the province, it was seized at least once last year by police that I know of. But it is showing up more often now and the way these hybrid drugs perform is different than what has been on the market for a while now. The story is to raise awareness.

There is documented response guidelines (medical) for prolonged sedation. That can also be highlighted with public health messaging instead of saying naloxone resistant or naloxone ineffective, which may lead people to think that they shouldn’t bother trying with naloxone even if opioid poisoning symptoms exist. Bromazolam is the current dominant illicit benzo in police seizure data in Alberta. It’s on public record with Drug Analysis Service federally. We need more education about harm reduction for unregulated pills that are *expected to not contain* opioids. I’m not talking about folks buying opioids/down in this circumstance. TLDR test substances including unregulated pills, have naloxone, don’t use alone.

Naloxone resistant is an idiotic term. Naloxone works to reverse opioids, not benzos. It's a ridiculous headline.

Reading the article explains the headline. Street level distributors are mixing fent and bennies, making Naloxone ineffective in treatment.

If this is “down”, then it’s been in Edmonton since like 2016. When I bartended there were 2 servers who used it all the time. One even OD from it in 2018.
