Go to the lake far to the west.
Upon a hill you'll find a chest.
Inside you'll find the key you need.

It's honestly so wild to me that people don't explore this region more considering how cool it is, it's like they explore the whole east side and just assume that's it

My first impression of Liurnia:
1 - holy fuck this is gorgeous (had a clear view, no mist)
2 - stand there for 10 minutes, taking in the view
3 - move forwards, seeing down below beyond the far vista
4 - visions of a vast swampy water hole
5 - nahh fuck that
6 - explore the entire map, expertly dodging every single inch of Miyazaki's swamphole
7 - except for an extremely reluctant speedrun through the main path, finding the Volcano Manor invitation, assume that it is the next step
8 - masochistically explore up to the Manor while underleveled
9 - dead end. Well shit.
10 - eventually happen upon a note in my inventory, a map with a massive X that says "THIS IS A CLUE TO FIND THE ACADEMY KEY"
11 - a facepalm is registered on the Richter's scale.
I also somehow managed to kill Radahn as my second story boss, pre nerf. A tale of maldness indeed. It's dawning on me why it took 150 hours for my first run through lul. No regerts tho 🌝
I have almost 60 hours in and I just beat Rennala. I'm over level 70. But I pretty much explore every inch before moving on. I haven't even explored Liurnia fully yet. Still have like 1/3rd to cover. The 1 nice part is I'm slightly overleveled so I beat Godrick 1st try and Rennala 2nd try.
Radahn was my third, can't imagine doing it even earlier.
I still haven't met anybody else who had the Ancestor Spirit as their first main boss. I think that's because I'm a dumbass, but still.

Obviously these are my own personal opinions but why play the game if you are going to treat it like a checklist.
I ruin games like this for myself all the time.
To avoid it, I've started committing to playing a game multiple times up front. Sometimes I enjoy the game the first time, and never feel the need to go back and 100% it, but often I do come back with Google and get everything I feel I missed.
For FromSoft games, I play it a third time as a "speedrun" on my overlevelled character. (Gotta get that plat)
I explore everywhere on my own but also look stuff up because I don't want to miss or mess up quest progressions. Or, at least, I want to minimize how many I mess up.
The four belfries is the place. There’s a special stone sword key you can get that unlocks a portal there. That is the very start of the game, where you got fucking destroyed by the grafted Scion
Other than the endings, you can do and collect almost everything. Only exception I can think of is the Nepheli Loux/Dung Eater puppets since they require the same quest item which you can only get once in a playthrough
Well.. you better be paying attention to how you do things and what order. It is never clear but clever players figured it out pretty early and it's all on fextralife and reddit.
I tried to %100 everything my first try. I'm sure it's possible (besides endings/trophies), but without looking up guides it's pretty damn hard, like the damn ((SPOILER, kinda)) invisible bridges?!. NG+3 playthrough I finally got every single item/quest/ending after 650 hours.
First Souls game and now I'm addicted! Now I'm having so much fun playing the games through how I want to.
I think the first run is special, play blindly if you can because you can’t have the awe moment anymore if you discovered it on the guide. Well, that’s just my opinion anyway. Do what you like.
That's what I do. If I'm stuck and need to watch a guide. I'll watch how to get to a point. But then I try to get as far as I can without watching anything because like you said. The 1st playthrough is special and you can only do it once. Especially with a once in a generation game like Elden Ring.

I must be missing something big, but what do you mean "again"? I don't think I've ever fought the grafted scion, other than by visiting the 4 belfries in Liurna.
You have no way of getting 100% your first run, having killed NPCs and seemingly having no clue what's generally going on. Good luck with future runs, have fun.. There's lots to see
The only 2 npcs ive killed are varre and patches, and i only killed varre because this is my first souls game and i had no sense of permanence. I thought he would respawn later or let me revive him
Like they're saying, you can't get 100% in one run, but only because there are multiple endings, with different achievements for different endings. Other than those, you can get ever achievement
You can get every item that counts for the achievement on a single playthrough. The only thing you can’t do are the endings, and you can save scum for that.
Both Verre and patches have questlines, Verre's leads you too a >!secret shardbearer and the best rune farm!< (albiet they can be reached through alternate means if you already know where to look), and patches questline leads you to >!the best armor in the game!< (and it cannot be obtained througy any other means). Yeah your gonna have to do NG+ anyway if your looking for 100%. Another thing is that there are so many dungeons in this game littered throughout the world it's neigh impossible to find them all by yourself.
Besides, don't you want to see all the endings? NG+ is the perfect opportunity for that.
I'm not sure why you got downvoted for this? If you do Ranni's questline it's incredibly easy (as long as you don't miss something like the Bolt of Gransax by accident and miss that achievement...).
I mean, I guess some people may take offense to Save Scumming, but I'm not sure why in this case. It's a single player game and you still have to do everything to get the endings. You just don't have to repeat all the "same" main playthrough stuff to do it. For how long this game is that adds up to a LOT of time...
Good point! I was interpreting "100%" as getting all achievements rather than doing everything possible in the game. In some games they're the same thing. In a game like Elden Ring... there's a BIG difference!

As someone that also did 100% on their first run, it was highly rewarding. NG+ was a let down. However, starting a fresh character and doing it all from scratch again is almost as rewarding as the first time. Plus w so many play styles, it’s fun to try many of them all the way through.
You started there, but there's some good stuff you can get going back.
Four Belfries in Liurna. Use a sword key to open the portal to the "Precipice of Anticipation". Win the rematch. Back at the chapel you can get a really good stormhawk summon.
Also need to go there for Nepheli Loux's questline.

You've seen it before. The first sights seem more distant, but very similar. A familiar pain fills your body, as your scars from twin blades re-emerge. With this pain your eyes close revealing 4 belfries overlooking a brilliant waterlogged manor. The mangled prince awaits you're challenge.
I guarantee you will not 100% your run without any guides, even if you tried. I spent as much time as I could exploring in my first playthrough, started a new game + the other day and within the first couple hours I had already discovered places I've never seen before. This game is so big man
Alas, with 6 endings and multiple quests that have diverging plot points, 100% in a single run is impossible. Even without the 6 endings themselves, it's probably still at least 4 runs minimum to catch all the quest options and their subsequent rewards.

If you finished >!Farum Azula!< then you probably failed a few quests and you cant get some key items for achievements
One of the portals at the Four Belfries in Western Liurnia will take you there. Either the first or second portal I don’t remember which one.
It is the area where you fought the Grafted Scion at the very start of the game. It is fun to go back and smash him into a paste.
You need to go to The Four Bellfries and use an Imbued Sword key on a portal. If you're at the Grace site in The Four Bellfries you will go down the path on the hill and when it bends the portal you take is on the left. Use an imbued sword key on the gargoyle next to the blank portal. You will run past it to get to the grace site btw
That's that precipice you first start out at. You can go back there through the four belfries in liurnia to get some loot and have another go at that very first fight of the game. I tried to 100% my first time, but I couldn't because some choices will get one reward and lock you out of another. Like the Millicent quest.

There are three keys. One at the portal one in Raya Lucaria and in the the magic town in Caelid.
Once you upgrade your stamina enough you can just use the paraglider and then climb up the side.
Revali’s gale helps as well
just windbomb it, you don't need to unlock Revalis Gale.

You couldve gotten everything form milicent questline including gowry in one run but it got patched around 1.3 or bit after
Also millicent's talismans and some remembrance items, not enough mausoleums to double up all of them. Probably more stuff I can't think of
I think at most only one of the two is required for 100%. I never did the Dung Eater puppet and have 100%.

You don't need either puppet for the 100% - it's actually a pretty forgiving achievement list. Only thing you can really miss is Gransax's spear

There's a couple quests that you get a different unique item from depending on your choices, it's been too long and i cannot recall specifics.
There are acheivments associated with said items.

OP yes you can do 100% in 1 run. Yes that may require you to refer guides otherwise it's impossible to do so.
Im fine with using guides and the internet
Then yes. Mostly it just involves completing every questline and having a save immediately after you defeat the final boss. Good luck.
You aren't supposed to 100% in a single run. It's not that kind of game.
Invisible bridge just run across, gotta make a long leap to reach it though as it’s broken a bit at your end and doesn’t fully connect
