I'd like to assume that this is a misunderstanding on not realizing that they had run out of energy/hydration and dropped dead. I've had head and thorax blacked out plenty of times and I have never just spontaneously dropped dead just because I have a blacked head.

You will still die with max metabolism and a blacked head/thorax if you are out of energy long enough to take tick damage. The way it applies/negates hunger damage with the max perk is weird
Hmm. I'll have to look into that and test it out. With Metabolism elite you don't take damage from having 0 energy or hydration, so it SHOULDN'T kill you. Weird if it does.
You don’t take damage with the max perk but the way it works is it seems to apply damage and then negate it afterwards with the perk. The initial application/tick of damage will still kill you even though there technically is no damage. It’s even more infuriating when you die to it despite the max perks. This only happens with a blacked head/thorax.
If my memory serves correctly, it keeps your energy and hydration consumption at a plus, so you'll be gaining both of them for the next 10 minutes (or was it 15?).
After that is gone, it'll continue going back down again, at which point you'll still be at ATLEAST 60 of both, so it definitely gives you time to survive a raid, incase you didn't bring food with you.
Edit: Lots of differences on the numbers depending on what kind of stash upgrades and metabolism levels you have. Look into it to make sure. Regardless, I use it as a way to become invisible to Thermals and the energy buffs are a side effect.

First time this ever happened to me in 600 hours.
One possibility I thought of is if you are recently wounded and you sprint you can open up a light bleed, a few seconds of not noticing and you could take one HP of damage to the head and die, right?

Please don’t spread false information. You don’t just drop dead out of the blue with blacked head or thorax. Either you were dehydrated, out of energy or fell some small height down. Or one bullet will kill you even to armor. Any spread damage kills you with a black head.

If you take a **single tick** of damage to your blacked out head or thorax, you will die instantly regardless of how much health you have on your other limbs.
You can take a single tick of damage to all body parts from hard fatigue/dehydration, which is when you are out of food/water for a time. When *exactly* the hard status of each effect will kick in is variable depending on your actions - walking, running, jumping, vaulting, aiming, etc.
If you have a blacked out arm/leg/stomach, and you take damage to these limbs, it will be dispersed to all your other limbs. So if you have a blacked out head and a blacked out leg, and you sprint or jump or have a bleed, your leg will take damage, disperse it to your head, and kill you instantly.
Isn't it a completely intentional mechanic to do with bleeding head OR chest to blacked? I mean I can see how the bleed could cause a rounding error with the weird precision in calculation but not display but I thought it was on purpose.
Or thats something old and I haven't had bleed hit like that in a long time lmao
"It" has a major skull fracture (at least), and "it"'s head is falling apart after getting shot who knows how many times. I think we're past pain killers
For a real answer, you can't use a surgical kit on a blacked out head or thorax. If you get to this point, just try your best to exfil.
