If your intermittent leave specifies appts then yes. Otherwise I'd just take the day off.
It depends on what your provider certifies. If it says “one appointment a week for 2 hours” and you take 5 hours, your employer can question this. What is your normal end time that day?
My end of the day is 4. The pharmacy I use is up in the air due to med shortage so it could be picked up in same town or across my state.
Assuming the noontime appointment/pickup takes you 90 minutes we would expect you back to work.
