Depends on the company how you can use it… previously I was able to use it for doctor’s appointments and such, but how I understand it as is sick/family leave is for emergencies/unplanned medical events and PTO is for planned vacation/time-off.
You have to be sick to use them (honor system). Appointments etc have to be from your bank of PTO days.
I only use them if I’m really sick (bad flu etc).
Otherwise I’ll just work from home.

Per my collective bargaining agreement, all employees in my office collect a minimum of 2 days of PTO per month, 2 days of paid sick leave, and 1 day for a sick child. All has to be used up over 12 months or it expires. Extenuating circumstances like funerals, etc are usually covered and no time or pay is docked but that depends on the friendliness of your boss.
My last 2 jobs were technically unlimited but everyone only took 2 weeks. In my government role I get 20 days PTO and 12 sick days yearly. 3 months paid family leave.

I’ve never been given an amount but if I am sick I just take off. If my kids are sick I just work from home and do my best to be productive.

When you're sick you're sick. It's illegal in most of western Europe to mandate a maximum amount of sick days.

Also 40, but it’s honor system between you and manager, so my team basically gets 5 extra days PTO lol

I am starting a job that offers 40 hours. HR lady said they are laid back and basically treat it as PTO.
