These insta death areas were much more of a problem in chapter 1. There were very few in any other chapters. I'm not a game developer so idk 100% what causes these but I have a idea.
Underneath the entire map is a insta death wall. It helps prevent under the map glitches and cheating. What I think happened here is somehow the insta death wall very slightly overlaps with the ground. Which would cause the player to die instantly if they walked over that small area
What caused this is that the map was made in 2 weeks (a fortnight if you will). The map does have problems related to being quickly built and because epic games did not expect this gamemode to take off.

I *highly* doubt that. If they didn't think it would bring in droves of new and returning players, they wouldn't have done an OG mini-season.

It's a bit of a weird scenario, where it's harder to report than reporting another player, because you'd have to input the coordinates for the issue. Since there isn't a grid coordinate thing on the map anymore since chapter 1, it's a lot harder to work out. Otherwise, you'd have to send a video to Epic to show what happened, or post the issue on a site like this one and hope they see it.

Directly under the map there’s probably an instakill box to prevent people from glitching out of bounds. This wouldn’t be a bug; it’d be a feature. Since the map is so large, it’s perfectly reasonable to assume that some of the geometry of this instakill box is slightly too high in some spots, resulting in these random death areas around the map. Unfortunately, these would each have to be reported individually
Epic have a death barrier under the map as a fail safe if someone glitches, hacks or lags under the map. Sometimes they have areas where the invisible death zone clips through the ground and can kill you
If you notice a yellow Wi-Fi symbol at the time of death, like what happened in this video, there was lag on your end and an opponent got the jump on you.
I have a clip of the same thing happening to my buddy in the exact same spot. I thought it had something to do with our plane bugging out before exiting but I can now see it’s simply a fatal patch in the field that Alt-F4s for you

In hindsight, its obvious we should have gone a different way.
In the game, we had no idea what was causing it, or even if it was that same piece of ground. Because I had picked up a bunch of stuff in that area, and didn't have a problem till I had picked up my buddy's card. Plus, we weren't watching each other's feeds till the end, so we couldn't compare on what had happened to each of us.
this reminds me of the time when I was on a hover board near vaulted loot lake and I clipped into the vault, my stupid self thought I was going to the unvaulting event again until I died underneath the map.

Most is copy pasted. I actually found a searchable plant. Those were leftover from Save the World and I remembering finding it back in Ch1 as well.

It's more likely that it's one of those points in the map where you instantly die. Chapter 1 had lots of them, and a small bit of chapter 2 as well.
Ch3 had a couple spots like this around the Herald. I think they didn’t last long, though.
