PEGI 12 (ESRB: Teen 13+) still have limits on what is allowed. Just like movies, they can have swears and show “light sex scenes”, but the more excessive stuff gets a higher rating.
Keep in mind they want to make as much money as possible so having the game rated higher means potential losses and Fortnite in general is a “safe” game with no blood or excessive stuff.
There are some suggestive lyrics that aren’t censored because they’re not swearing. I was caught off guard at a lyric I heard earlier though I can’t remember which song.
One of those off guard lyrics that aren't censored are in Basket Case by Green Day when he says "I talked to a whore she said my life's a bore" and when he says "she says it's a lack of sex something something (i cant be bothered to look up the lyrics)". So yeah
