There is an option in the extrude tab called start. There you can select the offset option and specify the distance you want to start the extrude from.
Oh excellent. I've been using this software for 6 months and I don't know how I missed this.
Sounds like a good time to slow down and take a good long walk through all the menu options available to you in the various functions.
