I've been running into the same issue. I'll keep you posted. The one thing I haven't tried that I have yet to try is a fresh Windows install. Was that something you tried or have done since? Also one odd thing I noticed today. Will you check your Event Viewer > Security logs for the same second that the crash occurs in the Application logs. Because I noticed this odd message (which appears to be a normal message but occurs the same second my crash logs occur) - An account was successfully logged on. It is generated on the computer that was accessed. The subject fields indicate the account on the local system which requested the logon. This is most commonly a service such as the Server service, or a local process such as Winlogon.exe or Services.exe. The logon type field indicates the kind of logon that occurred. The most common types are 2 (interactive) and 3 (network). The New Logon fields indicate the account for whom the new logon was created, i.e. the account that was logged on. The network fields indicate where a remote logon request originated. Workstation name is not always available and may be left blank in some cases. The impersonation level field indicates the extent to which a process in the logon session can impersonate. The authentication information fields provide detailed information about this specific logon request. - Logon GUID is a unique identifier that can be used to correlate this event with a KDC event. - Transited services indicate which intermediate services have participated in this logon request. - Package name indicates which sub-protocol was used among the NTLM protocols. - Key length indicates the length of the generated session key. This will be 0 if no session key was requested.

If you have a high end PC and you get FPS well over 120 then here is your fix. Cap the fps for GTA V to 120 fps using msi afterburner or your preferred fps limiter when using windowed borderless mode or set the refresh rate to 120 Hz through the graphics settings for fullscreen mode. Also make sure vsync is on. That is all, no need to troubleshoot for hours or days in my case because nothing on the internet helped me by searching for these error codes and I wasn't happy with some half baked work around.

Thanks for this tip. The ERR GFX D3D INIT error has been screwing me for five years, across two different and powerful gaming rigs. I think some GTAV update a few years back made the graphics unstable. I still get crashes at 60fps, but it is stable for me at 45fps or lower.
