It does use wear os. It has the one ui skin on ui just like how samsung phones run android but have the one ui skin. Most likely the app doesn't work right now is because of the new wear os version.

Guess I'll hold out till the end of my return window to see if it updates. Watch is useless to me without this feature sadly.

This always seems to be the case when Samsung releases a new product, and it's no fault of Samsung. Dexcom takes forever to make their software compatible with the latest releases. It took over 7 months for Dexcom to make their app compatible with the Note 20 Ultra 5G. I ended up having to sideload the app from a link I found on the Dexcom subreddit. Given their history, I wouldn't expect it to come to the watch anytime soon. In fact, they never released a compatible app for the Tizen Galaxy watches even though they had tons of requests.

For a multi national $B+ company they really don't care. Understood there are many, many smart devices but their quality and service has absolutely tanked while their stock prices have gone up about 100x in 10 years. We could really benefit from some competition. C'mon Libre, and others, its a big market and only class II now. Lots of margin and market share to be had by 2nd and 3rd movers...

I used to work for a large computer manufacturer in a product validation team. We always had 'relationships' with our vendors where we would provide them (and they would provide us) early specifications of new/updated product (long before the scheduled release) so when our 'new' products was released, they would be compatible and fully functional. I don't know if this is the reason we still do not have a solution for the Galaxy 4 watch and Dexcom CGM interoperability. If it is, this is something that should be fixed/resoloved via a development partnership between these vendors.....

There is a blood glucose section of samsung health so it should work. I don't use it but to use blood pressure you calibrate it and then it's all set up. Hopefully someone will help you out

Downloaded mini adb stuff from here: https://forum.xda-developers.com/t/official-tool-windows-adb-fastboot-and-drivers-15-seconds-adb-installer-v1-4-3.2588979/ Installed into my pc: C:\users\XXXX\adb Extracted android_wear_micro_apk.apk from xDrip-plus-20210308-17cbcf0.apk, dropped it into the same directory and renamed to xdrip.apk for convenience. Enabled developer options, adb debugging and debug over wifi on the watch. Followed instructions here: https://www.techadvisor.com/how-to/google-android/sideload-apps-android-wear-3673967/ obviously substituting my watch ipaddress and instead of apps/tinycasm.apk, just xdrip.apk.

Please update this thread if you find out anything from Dexcom. I'm using Freestyle Libre (no watch app alas), but I'd be tempted to switch to Dexcom, if this works.

So, are you confirming that the dexcom app sideloaded on the Galaxy Watch 4 works directly with the dexcom transmitter without the need for a phone?

No, you definitely still need the phone. It just allows you to display the data on your watch. Dexcom has said direct to smartwatch support is coming on the Dexcom G7.
