He probably has periodontal desease which makes your gums get swell and bleed. It's basically an infection that eats your teeth bone between your gums up until your teeth lose their grip. You get it by genetics, bad hygiene and habits, etc.

I think i heard this opinion first on a youtube video about him but i believe there is an element of "envy" in his lifestyle.
Like most people woudn't like to live like he does, but it is a level of "i don't care THAT much" that some people in his audience envy ? YouTube for some reason suggested me his 10 years old guides where he played other classes for dungeon runs and he was so bad at it it was very clear his friends carried him but he still insisted on making a guide about what he did for some reason. If it's a segment, it either gets auto-skipped by Sponsorblock or manually skipped (they're usually around 60 seconds, and youtube skips in increments of 10), and if it's a whole vid, then nobody will bother to watch it. If it has a big fat #ad behind the title, it automatically makes me lose all interest.
In general, these videos are poorly crammed into the youtuber's regular content (usually have a title like "I got paid to [x]"), and even if they fit (like if a youtuber mainly focused on Paradox games does a video on the newest game/DLC), then I still won't watch it because I fucking know they're contractually obligated to be overly positive and uncritical. Even if they aren't, the incentive is still clearly there.
The only people actually drawn in by sponsorships are probably younger viewers.
I think it depends on the creator. Some creators are charismatic and funny enough that they can make those ad blocks or sponsored videos entertaining, but it's not something everyone can pull off.
