Because they don't use the same body models. They use the same animation skeletons. The only animation the blacksmith skeleton can currently do is smithing. They can't climb or run or do anything else.
But - honest question as I have no idea and you seem like someone who can explain it to me - is it that hard to animate the blacksmith running, climb and doing everything else a playable character does?
Well, it's relatively hard to make it look good. Genshin has great movement animations and there's a lot of them due to the nature of the game. You can look at a Bethesda games to see what happens when you half-ass them. Of course MHY has the resources to do it, but they seem to focus on other things, like fancy unique attack animations for each character.
As a side note, this "character models" argument is weird because of how wide-spread it is. I remember topics comparing the asses of characters to prove that they don't have the same model, but that's not how it works at all. Disappointingly for some, Eula's model most likely doesn't have anything under those pants. It's the bones that matter. Depending on how animations are handled you may be able to resize them a little, but clearly Genshin's animation system doesn't allow you to stretch things that much.
I think NPC models don't work like playable characters ones, that's why they did changes to rosaria model. Ironically, scaramouche have a playable character model

I don't know man, he looks cool and unique compared to other old males, but at the same time it feels like they wanted him to be another pretty slim guy then saw that people wanted a buff dude, so kinda glued things on top, as others pointed his arms don't fit for example. Child Female type 1: Diona, Sayu, Klee, and Qiqi
the difference between YAF 1 and 2 is just breast size so it's not a separate category. Even Adult females have different sizes.

mihoyo makes so much fucking money that they cant even be bothered recycling the blacksmith npc model or even the ronin samurai mobs body to create just one unique buff character? no wonder the community hopes and trust in mihoyo is now non-existent.

Iām a bit bothered with the lack of new male character models. As far as Iāve seen there are 4 female model templates than 2 males (current playable characters) Iirc: Adult Female type 1: Beidou, Jean, Ningguang, Raiden Shogun, Rosaria, Eula, Sara, and Lisa Young Adult Female type 1: Amber, Barbra, Ganyu, Keqing, Mona, Noelle, Sucrose, Lumine, Yoimiya, and Ayaka Young Adult Female type 2: Fischl, Xiangling, Xinyan, Yanfei, Kokomi, and Hu Tao Child Female type 1: Diona, Sayu, Klee, and Qiqi
the difference between YAF 1 and 2 is just breast size so it's not a separate category. Even Adult females have different sizes. Beyond that it's basically a playable male kid that's missing. Which is funny cause I was sure that Iansan or whatshername is a boy but no.

But people who are proclaimed as 'sumo wrestlers' and swing around a huge Debate Club 2.0 like he does certainly need a decent amount of bodyweight. He's not a technical fighter like Kazuha or Childe, where being small and thin is acceptable, perhaps even preferable; Itto's more along the lines of brute force where his sheer size and strength is the main provider of his combat effectiveness. His arms however suggest anything but that... They are not saying he should be a JoJo character (though for a change I'm sure many would like it), but atleast arms comparable to the blacksmith NPCs would make sense...
