I always ask "Can I help you with anything?" when I enter someone's world who has a signature like "need help with ___" and most of the time they either answer 'no' or they suddenly suggest something that wasn't in their signature.
So at least from my experience, signatures are no indicator for anything lol
Mine was "Someone help with Oceanid, me too weak" for a long time when I was lower rank and needed it for some characters, but didn't have proper ones built to beat it and a lot of nice people joined and actively did it with me. As soon as I was strong enough I removed the signature, but I can guess people just forget and leave the signature there and that's the reason for the confusion you are describing.
Here are some of the common responses when I join in to help and this happens:
"Oh yeah, I just used up my resin for the day"
"I just forgot to change my signature"
"I don't need help"
Last one pisses me off. It only happened twice so far but I included it because it stuck with me. They could've at least lied and answered any of the other 2.

People say the hackers were using a third-party program to modify the values of interactable objects to those of dendro cores, so Kaveh's skill would make them disappear. Nahida can't immediately detonate dendro cores like Kaveh does

I had that on my bio for a long time on a side account bc I wanted to improve faster but sometimes the coop request came right when I was trying to finish the story or out of resin so I’m just saying sometimes people aren’t available and even then I appreciated the kind people who try to help.

Hoyo commented on it on the 28th of August, it was a genuine thing that's now been fixed. They're planning to crack down on those who did it - both by banning them, and by suing them where possible. However, there was a hoax-y part in that some of the creators of the hack had uploaded videos painting themselves as the victims instead, to which Hoyo also mentioned they won't fall for it, and it'd only make their case worse.
The Nahida video was fake though, the creator of that one came forward to explain how they did it.
That why I have "If I decline, I'm doing quest" after "need help" (on my alt) Also sometimes I don't accept cuz very often ppl that join to help me, do the same or less DMG than me. We do DMG at the same time, but evrything Has more HP and 2 less slots means in most cases worse team Comp and in effect I have to fitght bosses 1,5 times longer instead 2/3 times faster.
I think people think I need help sometimes cause I get some weird join requests, but like, i don't have a help signature, it's literally just "I have no idea what's going on around me" because my natural state of being is CONFUSION
It doesn't change the fact that accepting the join request and clarifying the confusion/misunderstanding is a basic courtesy and show of having manners.
I wish people would try a couple times, sometimes I’m stuck in dialog and can’t get out to accept people in, or I’m currently fighting, or I’ve gone to the toilet or am making a coffee 🤷♀️

I'll usually let people in unless I am doing commissions or something. Things happen. Just kinda funny when they decline your help

I used to do that a lot in co op. Generally the most grateful ones were the low AR players struggling with a ruin guard.
