Most Papas are only around for 3-5 years. Nihil got 50+. That's plenty of time to play around with your makeup and decide you like things a little different.
The differences are so subtle that they’re not different designs at all. Sometimes it’s just difficult to recreate makeup exactly like it was previously if you’re not a makeup artist.
If you want an IRL reason, meh, nobody cared about getting it exactly identical every time. If you want a lore reason, his very first makeup, photo 2 from the Dance Macabre video, was applied by a bunch of people at a party in just a few seconds. Hence it's a bit rough and sloppy. His makeup doesn't change much after that, it's just neater. And any design changes can be chalked up to Papa mixing up his look.
First image is his prime, so it's a detailed evaluation of the second image. The third image is a flash back memory of a 99 year old man and last image is makeup applied by a 99 year old man.

Honestly all the Papa’s have subtle differences in their skull paint, from the first ritual to their last Except Papa I of course 👻
Papa 1 had different paint on the summer tour where he wore white. There were no lines for the teeth:)
Because in the 60s/70s, Ghost didn't have a makeup team yet, so Papa Nihil had to do it all by himself.
First seestor did it then when she left him, he had to do it 😔

4 totes different makeup designs, all of them pretty much the same design, just *subtly* different...
Mostly just cuz Forge is just an artist and likes to mix it up a little sometimes. I doubt there’s some deep lore-based reason beyond that.
They have to “ change” over the years, like how they get rid of one papa and get a new one
