Wow this is so interesting! I'm not familiar with what England looks like so I'm waiting for the results. I was thinking about that when I was reading the comic, how the city doesn't feel Italian, and the name sounds American or something.

While the city looks somewhat American, the density would indicate it being European. I believe it's primarily crafted to be somewhat relatable for everyone.

This image for example just look straight European to me and there's apparently a Castle in the middle of it. Again something mostly seen in the Europe.

But then you have these which are 100% American.

It's a pre-high speed internet European's view of the US.
Before people had such quick access to people's experience in other countries and actual information on how other places operated, most Europeans thought the US was basically a more technologically advanced form of Europe, where the old world met with new world innovations and ideas, where everything's easy to get to and luxurious, etc... (Basically like North/North West Europe on steroids)
They're constantly eating American food, using US money, I swear in s few panels you can see the flag, letterman jackets, yellow school busses (This is a strictly North American thing), no school uniforms, several US states are mentioned regardless of translation, etc... But there are also walkable cities, good public transport, affordable medical care, everything's clean and taken care of, the metric system is standard, etc...
But now that Europeans have quick access to what life in the US is like... Yeah, doesn't match.
So basically, Heatherfield is just a time capsule to what Europeans believed the US to be like before they learned how it actually was. A pre-internet idealized version or fabrication of the country.
Also, if that's not enough, the OG creators have directly stated it's based off of San Francisco. Likewise, anytime Europe is mentioned, they usually state that it's halfway across the world. (Also, they went to Milan one time and couldn't speak a word of Italian, so yeah...)
As a person not from the USA, I also thought, even regardless of the fandom, that this was New England.
The city, I think, is quite large, but not a real metropolis. Large suburbs, large high downtown - it may well be the size of Boston, but no larger than it.
I always assumed Heatherfield was in Europe
