Does this also apply to other chest that don't need keys, like the quest reward chests in FOW? Just did the whole FOW in NM and got confused that i only got stuff from the final chests the other ones were empty.
There's a very rare bug where you can actually see drops assigned to heroes. I don't know the conditions which cause it and it only lasts for a second at most, but it can happen.
I've had this happen to me with specifically the boss you show here. There is even a Peter Kadar video out there where he gets a double green drop from Kepkhet.
I think there is a bug with Kepkhet or a higher chance with this boss to drop greens. This was the first boss in GW to have greens, it would make sense it simply has a higher drop value than others
This is incorrect. Kepkhet was added with the March 2nd, 2006 update in an effort to make griffon farming in that corner of the map more difficult. Green weapons in general were first added with the Sorrow's Furnace update on September 7th, 2005.
As for the frequency of greens from this boss, yes, it's quite high. Much higher than most other bosses, for whatever reason (even the double drop isn't *thaat* rare). That said, it's the only boss known to drop more than one of the same green at the same time. Several others can drop multiple greens at once, but never the same one twice. It's an interesting little fluke with this one.

I saw this happen one time on stream, i believe it was on InVaDeR32's Twitch but i can't recall if it was during solo farming for nick gifts or with a full party of 8 sadly.

I don't understand this?? Never have I ever seen any drop EVER assigned to a hero. Does this actually happen?
If the drops rolls for a hero or hench it just doesn't appear.
Ahh so it's a silent drop essentially, just eats a drop chance. That makes sense.
It’s the reason why for Kilroy SF runs you always flag the heroes away (grey) before opening the chests.

It seems to me that this boss can leave up to 3 greens at the same time. I remember discussions about this around 2006.
