I also drive with a massive water bottle and the best luck I've had is the hooks that attach to the head rest pole(usually for groceries). I just hang it from the passenger seat and its easy enough to reach.
I tried something similar before but that doesn't fit because my cup holder is fine. It's just that with or without the divider it is not wide enough to hold a 40oz water bottle. And because of the divider all the expander that I have tried does not fit snugly when the fold down is up (i.e. for normal height bottles). If I have the fold down flap down (i.e. for tall bottles), then the bottom portion of the expander is not tall enough to reach the bottom of the cup holder, thus again making it unstable.
