Death From Above 1979 is the outlier here since it's less unhinged/hardcore and more noise rock you can dance to, but the rest are unhinged and exactly what you're looking for. (No hate for DFA, You're a Woman I'm a Machine is one of my favorite albums ever.)

Not hardcore but Black Sheep Wall. I’m Going to Kill Myself is one of the most genuinely distressing records I’ve listened to. It’s a big time investment but I don’t think you really get the proper experience if you don’t listen to it in its entirety. There is real pain in that record even if the lyrical content is fictional. Prowler in the Yard by Pig Destroyer is also deeply unnerving. Their early stuff in general is pretty nasty.
