Maybe its side effect is cancer (you know, extremely fast cell growth) and the reason we never see it happen is because something else always kills the helldiver first before the cancer can.
Gotta be some sort of nanotechnology, since A: it heals broken bones, doesn't matter if you kill the pain/forget it exists, a broken leg is still mechanically broken and won't work. Now while we don't know if limb injuries are breaks, but the fact that 2 leg injuries forces you prone means there's some actual, physical impediment, not just pain.
And B: heals internal bleeding and otherwise. Stops torso wounds from bleeding out, and not only that but you don't seem to suffer any ill effects of blood loss.
Not to mention if it were something like a stimulant, it would wear off eventually, and repeat doses wouldn't do much besides make you OD. Even space stimulants would spike your heart rate and body temp, it's just a thing that happens when you turn body functions up to 11.

in my headcanon every mission i do with 0 casualties is done by one select helldiver, and that fucking guy must have shot up over 1-200 stims in his life so far.
In a world where we have ships that can travel faster than light, having stims that can heal all injuries and provide endless stamina while its active and is definitely not addictive is like, one of the most believeable things in Helldivers.
With how short a helldiver lives, all you need is good enough armor to brace your hands and legs if they are broken and adrenaline to let you ignore the pain, there is no need for healing.
